Please ask our receptionist at the surgery about registering with the Practice. She will check that you live within our practice area and give you a New Patient Registration form and questionnaire to complete.

Once completed, please return the forms and make an appointment to see the Health Care Assistant for a New Patient Check. Please bring a urine sample with you to that appointment.

Please note that you will have to see a GP / Nurse if you are on repeat medication.

Now you are able to register with the practice online as well. Please follow the link below which takes you to a secure website to provide initial registration information to us. Please fill in all the boxes to provide us with enough information to register you with the practice. The information you provide online will be reviewed by our registration administrators and if you are in our practice area, you will be registered. You will still need to complete the New Patient registration form and questionnaire to complete the registration. 

Once you have done the online registation we will contact you to confirm if you have been registered with the practice and invited to have a initial health check appointment with our Health Care Assistant. This appointment is an opportunity to have a health check, and helps you to develop a relationship with the practice.

Please note if you are on regular medication from your previous doctor then you might need to be seen by our nurse / doctor to make sure all your information is updated on the record. It can take a while to get your records from your previous GP, hence it is best if you make appointment for an initial assessment and we can update your record to make sure you get all your medicines and checks in time.
